Most individuals with type II diabetes feel perfectly fine, but unnoticed damage can occur over time. The A1C test, which is a common diabetes test, only measures your blood sugar level over the last 2-3 months. This is why continuous management matters – it’s crucial to manage your diabetes proactively, even when you’re not noticing symptoms.

Our program consists of a minimum of 12 appointments over a two-year period. At the core of our program is food and nutrition science. We also incorporate evidence-based research and will ALWAYS provide 1:1 support. We focus on building your unique food and exercise program. Requirements for Acceptance
Our program is available to patients diagnosed with diabetes and/or CKD Stages 1-5. We require patients to secure their labs from their Primary Care Provider (PCP).
Cost and Insurance
We work with all insurance providers, including state Medicaid programs. We accept Humana, Essence, Mercy, Aetna, United Health Care, Blue Cross Blue Shield, Medicaid, traditional Medicare, and all Medicare Advantage plans. Any required co-pays will be communicated as early as possible.
